Explaination: Menstrual migraine attacks are due to estrogen withdrawal during menstruation. Although estrogens are neuroexcitatory, they appear to have a protective role in migraine. Perimenstrual migraine attacks are more severe and refractory to treatment than non menstrual attacks. These attacks are more commonly migraine without aura. Treatment strategies include using Naproxen 500 mg bid or naratriptan 2.5 mg bid starting two days before expected date of menstruation till day three for a period of 5 days.
