Treatment of acute spinal cord infarction is primarily based on efforts to improve spinal cord perfusion. Hemodynamic augmentation including use of vasopressors toincrease the perfusion pressure is combined with lumbar drainage of CSF to reduce the resistance to blood flow. Robust controlled clinical trials to support these measures do not exist, but the recommendation is based on observations of occasional dramatic improvement with these approaches.
